How Long Is the Border Wait, and How Do You Plan an Appointment Around It?

The two directions are not the same problem

Crossing south into Tijuana is, on almost any ordinary day, a matter of minutes. There is no meaningful inspection queue for vehicles heading into Mexico, and a trip from the San Ysidro port of entry to a Zona Río clinic normally takes fifteen to twenty minutes door to door.

Crossing north into the United States is where the hours live. Every vehicle is inspected, the queue forms on Mexican streets, and the wait swings between twenty minutes and three hours depending on the day of the week, the hour and whether it is a holiday on either side of the line.

The pattern, roughly

Weekday mornings northbound are heaviest between 5:00 and 9:00 AM, when the commuter flow crosses to work in San Diego. Mid-morning to early afternoon is usually the calmest window of the working day.

Late afternoon builds again from about 3:00 PM, and Sunday afternoons and evenings are consistently among the worst of the week as weekend travellers return. Holidays on either calendar — American or Mexican — distort everything.

For a patient, the practical consequence is simple: a procedure that finishes at 4:00 PM on a Friday puts you in the worst possible line at the worst possible moment. That is worth knowing when you schedule the surgery, not when you get into the vehicle.

What the fare includes, and what it does not

Our published fares include one hour of border wait in the regular lane. Beyond that hour, additional waiting is billed at $15 USD per hour, which is not a penalty but the cost of a vehicle and a driver being stationary rather than working.

In practice, most northbound trips fall inside the included hour. The ones that do not are almost always the Friday-afternoon and Sunday-evening crossings, which is exactly why we recommend the medical lane on those days.

Building a day that holds

Work backwards from the appointment, not forwards from your hotel. A 7:00 AM surgical check-in in Zona Río means leaving downtown San Diego around 4:45 AM, and that is with southbound conditions being easy.

Then plan the return as a separate problem with its own margin. If the procedure has an uncertain end time — most do — book a return window rather than a return minute, and keep dispatch informed on WhatsApp while you are still inside.

And if the patient will be sedated or in pain on the way back, treat the medical lane pass as part of the medical plan rather than as an upgrade. Two hours in a queue is a different experience for a healthy driver than it is for someone six hours out of an operating room.
